The Pulaski County School District has decided to participate in the Kentucky Incentives for Prevention (KIP) Student Survey. The survey is funded by the Kentucky Division of Behavioral Health with the support of the Governor's Office of Drug Control Policy and the Federal Center for Substance Abuse Prevention. It is designed to assess alcohol and drug use among students in grades 6, 8, 10, and 12. The survey is completely voluntary and will be used for school planning and program development.

The Pulaski County School District is participating in this important project because we believe young people's use of these substances is a serious issue for our communities. We encourage you to support this project by agreeing to let your student participate in the survey. If you have any questions, please don't hesitate to call your student's school coordinator, Annie Wheeldon at 606-676-0164. Thank you for your cooperation.

Please fill out this form only if you DO NOT give permission for your student to participate in the survey by October 15, 2021. The KIP Survey will be administered to all PCHS 10th and 12th grade students on October 21st.
